We strongly believe that medical and pharmacy claims do not tell a full story. Our first step is to speak with the patient and explain the Member First program. Next, we collect pertinent information about the patient during our clinical interaction.
No two patients are the same. We assess every patient as a unique individual. This is an interactive process where our highly trained, board-certified clinicians engage with patients to gather key information metrics such as drug administration schedule, for example.
Based on our clinician’s confidential, one-on-one encounter with the patient, a medication action plan is created that is customized and specific to each patient’s needs.
Once the medication action plan has been agreed upon between our clinician and patient, the plan is implemented. This entails provider engagement as needed.
patients who are actively enrolled in our Member First program receive regular follow up appointments to ensure successful implementation of the individualized patient’s medication action plan, including the ongoing review of healthcare claims.
